Fall 2012 Kindergarten registration for Greenwood Lake School District begins Feb. 6

    Greenwood Lake — Registration of children who will enter Kindergarten in September 2012 will take place Monday through Friday, Feb. 6 through 10, from 9 a.m. to 3 p.m. each day. Children entering Kindergarten must reach the age of 5 by Dec. 1, 2012. Parents may register their child at the Greenwood Lake Middle School Office at 1247 Lakes Road. Registration packets are available at the Greenwood Lake Elementary School, Greenwood Lake Middle School or online at http://gwlufsd.org/. When registering your child, bring a completed registration packet and the following information: Original birth certificate, proof of residency and certificate of immunization. Certificate of immunization must be prepared and signed/stamped by a physician or authorized person who administers the immunizing agent and shall specify the vaccines given and the dates of administration. Children are required to have the following immunizations: 3 doses of Diphtheria Toxoid-containing vaccine 3 doses of Tetanus Toxoid-containing vaccine and Pertussis vaccine (DTaP, DTP) for children born on or after 1/1/2005 3 doses of Polio IPV or OPV Measles, Mumps and Rubella - 2 doses of measles-containing vaccine and 1 dose each of Mumps and Rubella (preferably as MMR) 3 doses of hepatitis B vaccine 1 dose Varicella (Chickenpox), diagnosis of Varicella disease by a physician, physician’s assistant or nurse practitioner is acceptable proof of immunity for school entry/attendance.
